Why do multi-agent LLM systems fail more than expected?

Despite growing enthusiasm for multi-agent LLM systems, performance gains across popular benchmarks often remain minimal compared to single-agent frameworks. MAST (Multi-Agent System Failure Taxonomy) is the first empirically grounded taxonomy designed to understand why.

The methodology is rigorous: 5 popular MAS frameworks analyzed across over 150 tasks, with 6 expert human annotators identifying and categorizing failures. The result: 14 unique failure modes organized into 3 overarching categories:

Category 1: Specification issues — failures rooted in how the task or agent roles are defined. This includes under-specified goals, ambiguous role boundaries, and mismatched capability assumptions.

Category 2: Inter-agent misalignment — failures arising from how agents interact with each other. Communication breakdowns, conflicting sub-goals, and coordination failures live here.

Category 3: Task verification — failures in evaluating whether the task was completed correctly. This includes incomplete validation, cascading error propagation, and inability to detect partial failures.

The taxonomy extends prior work significantly. Why do autonomous LLM agents fail in predictable ways? identified 4 LLM-specific failure modes from a single framework (CAMEL). MAST systematizes across 5 frameworks and identifies 14 modes — providing broader coverage and empirical grounding.

The finding aligns with Anthropic's design principle that simplicity and modular components outperform overly complex frameworks. But MAST provides the systematic evidence for WHY: each failure mode represents a specific point where complexity introduces failure rather than capability. The taxonomy enables targeted mitigation rather than blanket simplification.

The practical implication: building robust multi-agent systems requires engineering against each failure category independently. Improving inter-agent communication doesn't fix specification issues. Better verification doesn't fix misalignment. The categories are orthogonal failure surfaces.
